The cause of the slump in economic growth is because of the decrease of household consumption, investment and government consumption. Sri Mulyani said that house hold consumption decreased from 3, 2% to 1, 6%. Its really bad effect many company in Indonesia, they have a slump income.

<li><strong>Cognitive Reappraisal and Self-Compassion as Emotion Regulation Strategies for Parents during COVID-19: An Online Randomized Controlled Trial</strong> -

Objective. Parenting during pandemic restrictions places extreme demands on everyday family life, leading to increased stress levels for parents and distressed parent-child interactions. This RCT aimed to investigate whether cognitive reappraisal and self-compassion are helpful emotion regulation (ER) strategies to reduce individual and parental stress during the COVID-19 pandemic. Method. An online intervention for parents was developed focusing on the application of ER strategies to pandemic requirements of families. A sample of 265 parents were randomly assigned to either cognitive reappraisal (CR; n = 88), self-compassion (SC; n = 90) or wait-list control (WLC; n = 87) group. Interventions included two video sessions (day 1 and day 3) and three email reminders to transfer the application of ER strategies to daily family life (days 2, 4, 5). Parents perceived individual stress and parental stress were assessed at baseline (T0), at T1 prior to the booster session on day 3, and at T2 (7 days after baseline). Results. Significant decreases from T0 to T2 emerged for both primary stress outcomes in both intervention groups. Individual stress significantly decreased in CR compared to WLC at T2, but not compared to SC. No time × group interactions for parental stress were found. However, mediation analyses suggested that parental stress was indirectly decreased via reductions in individual stress for CR compared to WLC at both time points. Conclusions. COVID-19 will not be the last pandemic to affect family life. Cognitive reappraisal as a brief online intervention can ease acute stress and strengthen the mental health of parents in acute crises.

The COVID-19 pandemic, caused by the SARS-CoV-2 virus, has become a global crisis. To gain systems-level insights into its pathogenesis, we compared the blood proteome and phosphoproteome of ICU patients with or without SARS-CoV-2 infection, and healthy control subjects by quantitative mass spectrometry. We find that COVID-19 is marked with hyperactive T cell and B cell signaling, compromised innate immune response, and dysregulated inflammation, coagulation, metabolism, RNA splicing, transcription and translation pathways. SARS-CoV-2 infection causes global reprogramming of the kinome and kinase-substrate network, resulting in defective antiviral defense via the CK2-OPN-IL-12/IFN-I axis, lymphocyte cell death via aberrant JAK/STAT signaling, and inactivation of innate immune cells via inhibitory SIRPA, SIGLEC and SLAM family receptor signaling. Our work identifies CK2, SYK, JAK3, TYK2 and IL-12 as potential targets for immunomodulatory treatment of severe COVID-19 and provides a valuable approach and resource for deciphering the mechanism of pathogen-host interactions.

Background: The COVID-19 pandemic has had a profound, negative impact on the lives and wellbeing of the population, and it can raise additional challenges for individuals with eating disorders. During early stages of the UK lockdown, individuals reported disruptions to many aspects of their lives, including reduced feelings of control and serious concerns over the impact of the pandemic on eating disorder symptoms and/or recovery. This study compares data from two time points to explore the ongoing impacts of the pandemic on this population. Method: A mixed-methods online survey was developed for the purpose of this study. Data was collected at the two key time points: First, soon after the start of the first UK lockdown (April 2020) and second, as the first lockdown restrictions began to be lifted (June 2020). The sample consisted of 58 individuals currently experiencing, or in recovery from, an eating disorder. Participants were aged between 16-65 years; 57 identified as female, and 1 male. Results: Higher perceptions of general, external control were associated with recovery between the time points. Individuals who experienced less perceived control reported a tendency to rely upon eating disorder behaviours as an auxiliary coping mechanism, i.e., diminished external control was directed inwards and replaced with controlling their own behaviour. Conclusions: Perceived control is a significant factor in eating disorder recovery. As a result of the pandemics negative impact upon peoples sense of control, individuals with eating disorders are at significant risk of detrimental impacts on their recovery and wellbeing. The results have implications for future treatments based on strengthening individuals perceptions of control to promote recovery.

<li><strong>Genetic screening for TLR7 variants in young and previously healthy men with severe COVID-19: a case series</strong> -

Advanced age, male sex and chronic comorbidities are associated with severe COVID-19. However, these general risk factors cannot explain why critical illness occurs in young and apparently healthy individuals. In the past months, several publications have identified susceptibility loci and genes using comprehensive GWAS studies or genome, exome or candidate genes analysis. A recent study reported rare, loss-of-function TLR7 variants in otherwise healthy young brother pairs from two families with severe COVID-19. We aimed to prospectively study the prevalence of rare X-chromosomal TLR7 genetic variants in our cohort of young male patients with severe COVID-19. We recruited 13 patients ≤50 years who had no risk factors known to be associated with severe disease. We studied the entire TLR7 coding region and identified two missense variants (p.Asn215Ser, c.644A&gt;G and p.Trp933Arg, c.2797T&gt;C) in two out of 13 cases (15.4%). These variants were not previously reported in population control databases (gnomAD) and were predicted to be damaging by all in silico predictors. The male index patients were between 25 and 30 years old and had no apparent comorbidities. The TLR7 p.Asn215Ser co-segregated in 2 first-degree relatives severely affected by COVID-19, in a younger previously healthy the variant was found in hemizygous state , and in an older than 60 was in heterozygous state. No family members were available for testing the segregation of the p.Trp933Arg variant. These results further support that susceptibility to severe COVID-19 could be determined by inherited rare genetic variants in TLR7. Understanding the causes and mechanisms of life-threatening COVID-19 is crucial and could lead to novel preventive and therapeutic options. This study supports a rationale for the genetic screening for TLR7 variants in young men with severe COVID-19 in the absence of other relevant risk factors. A diagnosis of TLR7 deficiency could not only inform on treatment options for the patient, but it also enables for pre-symptomatic testing of at-risk male relatives with the possibility of instituting early preventive and therapeutic interventions.

Coronaviruses have caused three major epidemics since 2003, including the ongoing SARS-CoV-2 pandemic. In each case, coronavirus emergence in our species has been associated with zoonotic transmissions from animal reservoirs, underscoring how prone such pathogens are to spill over and adapt to new species. Among the four recognized genera of the family Coronaviridae (Alphacoronavirus, Betacoronavirus, Deltacoronavirus, Gammacoronavirus), human infections reported to date have been limited to alpha and betacoronaviruses. We identify, for the first time, porcine deltacoronavirus (PDCoV) strains in plasma samples of three Haitian children with acute undifferentiated febrile illness. Genomic and evolutionary analyses reveal that human infections were the result of at least two independent zoonoses of distinct viral lineages that acquired the same mutational signature in the nsp15 and the spike glycoprotein genes by convergent evolution. In particular, structural analysis predicts that one of the changes in the Spike S1 subunit, which contains the receptor-binding domain, may affect flexibity of the protein and binding to the host cell receptor. Our findings not only underscore the ability of deltacoronaviruses to adapt and potentially lead to human-to-human transmission, but also raise questions about the role of such transmissions in development of pre-existing immunity to other coronaviruses, such as SARS-CoV-2.

During the COVID-19 pandemic, people have reported experiencing anxiety in response to the viral epidemic. This study aimed to explore the validity and usefulness of the Stress and Anxiety to the Viral Epidemic-6 items (SAVE-6) scale for measuring the anxiety response to the viral epidemic of the general population. A total of 1,009 respondents participated in an online survey, and 501 (49.7%) participants were rated as having at least a mild degree of anxiety response to the viral epidemic (SAVE-6 score ≥ 15), whereas 90 (8.9%) and 91 (9.0%) were rated as having depression and anxiety, respectively. The SAVE-6 scales showed good internal consistency (Cronbachs α = .82). Confirmatory factor analysis supported a one-factor structure for the measure. Goodness-of-fit indices (χ2/df ratio = 19.1, CFI = .92; TLI = .86; SRMR = 0.05; RMSEA = .13) were adequate. The SAVE-6 was found to be a reliable, valid, and useful brief measure that can be applied to the general population. The SAVE-6 may be useful for easily assessing the anxiety symptoms during the pandemic in the general population.

Genetic variants of the SARS-CoV-2 virus have become of great interest worldwide because they have the potential to detrimentally alter the course of the SARS-CoV-2 pandemic, and disease in individual patients. We recently sequenced 20,453 SARS-CoV-2 genomes from patients with COVID-19 disease in metropolitan Houston (population 7 million), dating from March 2020 to early February 2021. We discovered that all major variants of concern or interest are circulating in the region. To follow up on this discovery, we analyzed 8,857 genome sequences from patients in eight Houston Methodist hospitals dispersed throughout the metroplex diagnosed from January 1, 2021 to March 7, 2021. This sample represents 94% of Houston Methodist cases and 4.8% of all reported cases in metropolitan Houston during this period. We discovered rapid, widespread, and preferential increase of the SARS-CoV-2 UK B.1.1.7 throughout the region. The estimated case doubling time in the Houston area is 6.9 days. None of the 648 UK B.1.1.7 samples identified had the E484K change in spike protein that can cause decreased recognition by antibodies.

<li><strong>The impact of COVID-19 vaccination campaigns accounting for antibody-dependent enhancement</strong> -

Background: COVID-19 vaccines are approved, vaccination campaigns are launched, and worldwide return to normality seems within close reach. Nevertheless, concerns about the safety of COVID-19 vaccines arose, due to their fast emergency approval. In fact, the problem of antibody-dependent enhancement was raised in the context of COVID-19 vaccines. Methods and findings: We introduce a complex extension of the model underlying the pandemic preparedness tool CovidSim 1.1 (http://covidsim.eu/) to optimize vaccination strategies with regard to the onset of campaigns, vaccination coverage, vaccination schedules, vaccination rates, and efficiency of vaccines. Vaccines are not assumed to immunize perfectly. Some individuals fail to immunize, some reach only partial immunity, and - importantly - some develop antibody-dependent enhancement, which increases the likelihood of developing symptomatic and severe episodes (associated with higher case fatality) upon infection. Only a fraction of the population will be vaccinated, reflecting vaccination hesitancy or contraindications. The model is intended to facilitate decision making by exploring ranges of parameters rather than to be fitted by empirical data. We parameterized the model to reflect the situation in Germany and predict increasing incidence (and prevalence) in early 2021 followed by a decline by summer. Assuming contact reductions (curfews, social distancing, etc.) to be lifted in summer, disease incidence will peak again. Fast vaccine deployment contributes to reduce disease incidence in the first quarter of 2021, and delay the epidemic outbreak after the summer season. Higher vaccination coverage results in a delayed and reduced epidemic peak. A coverage of 75% - 80% is necessary to prevent an epidemic peak without further drastic contact reductions. Conclusions: With the vaccine becoming available, compliance with contact reductions is likely to fade. To prevent further economic damage from COVID-19, high levels of immunization need to be reached before next years flu season, and vaccination strategies and disease management need to be flexibly adjusted. The predictive model can serve as a refined decision support tool for COVID-19 management.

<li><strong>Arginine Methylation Regulates SARS-CoV-2 Nucleocapsid Protein Function and Viral Replication</strong> -

Viral proteins are known to be methylated by host protein arginine methyltransferases (PRMTs) playing critical roles during viral infections. Herein, we show that PRMT1 methylates SARS-CoV-2 nucleocapsid (N) protein at residues R95 and R177 within RGG/RG sequences. Arginine methylation of N protein was confirmed by immunoblotting viral proteins extracted from SARS-CoV-2 virions isolated by cell culture. We demonstrate that arginine methylation of N protein is required for its RNA binding capacity, since treatment with a type I PRMT inhibitor (MS023) or substitution of R95K or R177K inhibited interaction with the 5-UTR of the SARS-CoV-2 genomic RNA. We defined the N interactome in HEK293 cells with or without MS023 treatment and identified PRMT1 and many of its RGG/RG substrates including the known interactor, G3BP1, and other components of stress granules (SG). Methylation of N protein at R95 regulates another function namely its property to suppress the formation of SGs. MS023 treatment or R95K substitution blocked N-mediated suppression of SGs. Also, the co-expression of methylarginine reader TDRD3 quenched N-mediated suppression of SGs in a dose-dependent manner. Finally, pre-treatment of VeroE6 cells with MS023 significantly reduced SARS-CoV-2 replication. With type I PRMT inhibitors being in clinical trials for cancer treatment, inhibiting arginine methylation to target the later stages of the viral life cycle such as viral genome packaging and assembly of virions may be an additional therapeutic application of these drugs.

<li><strong>Factors Associated with Emerging and Re-emerging of SARS-CoV-2 Variants</strong> -

Global spread of Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) has triggered unprecedented scientific efforts, as well as containment and treatment measures. Despite these efforts, SARS-CoV-2 infections remain unmanageable in some parts of the world. Due to inherent mutability of RNA viruses, it is not surprising that the SARS-CoV-2 genome has been continuously evolving since its emergence. Recently, four functionally distinct variants, B.1.1.7, B.1.351, P.1 and CAL.20C, have been identified, and they appear to more infectious and transmissible than the original (Wuhan-Hu-1) virus. Here we provide evidence based upon a combination of bioinformatics and structural approaches that can explain the higher infectivity of the new variants. Our results show that the greater infectivity of SARS-CoV-2 than SARS-CoV can be attributed to a combination of several factors, including alternate receptors. Additionally, we show that new SARS-CoV-2 variants emerged in the background of D614G in Spike protein and P323L in RNA polymerase. The correlation analyses showed that all mutations in specific variants did not evolve simultaneously. Instead, some mutations evolved most likely to compensate for the viral fitness.

<li><strong>New detection of SARS-CoV-2 in two cats height months after COVID-19 outbreak appearance in France</strong> -

Although there are several reports in the literature of SARS-CoV-2 infection in cats, few SARS-CoV-2 sequences from infected cats have been published. In this report, SARS-CoV-2 infection was evaluated in two cats by clinical observation, molecular biology (qPCR and NGS), and serology (Microsphere immunoassay and seroneutralization). Following the observation of symptomatic SARS-CoV-2-infection in two cats, infection status was confirmed by RT-qPCR and, in one cat, serological analysis for antibodies against N-protein and S-protein, as well as neutralizing antibodies. Comparative analysis of five SARS-CoV-2 sequence-fragments obtained from one of the cats showed that this infection was not with one of the three recently emerged variants of SARS-CoV-2. This study provides additional information on the clinical, molecular, and serological aspects of SARS-CoV-2 infection in cats.

<li><strong>Oxygen provision to severely ill COVID-19 patients at the peak of the 2020 pandemic in a Swedish district hospital</strong> -

Oxygen is a low-cost and life-saving therapy for patients with COVID-19. Yet, it is a limited resource in many hospitals in low income countries and in the 2020 pandemic even hospitals in richer countries reported oxygen shortages. An accurate understanding of oxygen requirements is needed for capacity planning. The World Health Organization estimates the average flow-rate of oxygen to severe COVID-19-patients to be 10 l/min. However, there is a lack of empirical data about the oxygen provision to patients. This study aimed to estimate the oxygen provision to COVID-19 patients with severe disease in a Swedish district hospital. A retrospective, medical records-based cohort study was conducted in March to May 2020 in a Swedish district hospital. All adult patients with severe COVID-19, those who received oxygen in the ward and had no ICU-admission during their hospital stay, were included. Data were collected on the oxygen flow-rates provided to the patients throughout their hospital stay, and summary measures of oxygen provision calculated. One-hundred and twenty six patients were included, median age was 70 years and 43% were female. On admission, 27% had a peripheral oxygen saturation of &lt;=91% and 54% had a respiratory rate of &gt;=25/min. The mean oxygen flow-rate to patients while receiving oxygen therapy was 3.0 l/min (SD 2.9) and the mean total volume of oxygen provided per patient admission was 16,000 l (SD 23,000). In conclusion, the provision of oxygen to severely ill COVID-19-patients was lower than previously estimated. Further research is required before global estimates are adjusted.

<li><strong>Characterizing the COVID-19 illness experience to inform the study of post-acute sequelae and recovery: a qualitative study</strong> -

<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom">
We aimed to characterize the variability in the illness experience and recovery process from COVID-19. We conducted 24 in-depth individual interviews with participants enrolled in the Long-term Immunological Impact of Novel Coronavirus (LIINC) cohort study in San Francisco, California from June through October of 2020. Participants were adults who had a previously confirmed positive SARV-CoV-2 nucleic acid amplification test result, had recovered or were recovering from acute infection, and underwent serial evaluations at our clinical research center. We purposefully sampled 24 English- and Spanish-speaking adults with asymptomatic, mild and severe symptomatic infection, including those who were hospitalized, and those with HIV co-infection. Half of our sample (50.0%) identified as Latinx/Hispanic and most of the participants were men (62.5%). We used thematic analysis to characterize the illness experience, recovery process, and mental health impact of experiencing COVID-19 and present clinical data for each participant. Emergent themes were: (1) across symptom profiles and severity, experiencing COVID-19 was associated with psychological distress, (2) among participants with symptomatic infection, the illness experience was characterized by uncertainty in terms of managing symptoms and recovery, and (3) despite wide-ranging illness experiences, participants shared many common characteristics, including health information-seeking behavior facilitated by access to medical care, and uncertainty regarding the course of their illness and recovery. <li><strong>Untangling the cell immune response dynamic for severe and critical cases of SARS-CoV-2 infection</strong> -

COVID-19 is a global pandemic leading high death tolls worldwide day by day. Clinical evidence suggests that COVID-19 patients can be classified as non-severe, severe and critical cases. In particular, studies have highlighted the relationship between the lymphopenia and the severity of the illness, where CD8+ T cells have the lowest levels in critical cases. In this work, we aim to elucidate the key parameters that define the course of the disease deviating from severe to critical case. To this end, several mathematical models are proposed to represent the dynamic of the immune response in patients with SARS-CoV-2 infection. The best model had a good fit to reported experimental data, and in accordance with values found in the literature. Our results suggest that a rapid proliferation of CD8+ T cells is decisive in the severity of the disease.

The streets are empty, but Joanna Despas is still cooking. Heres what its like to run a food cart during a pandemic.

When I walk up to Mama Jos Breakfast Cart, which is located two blocks away from the MetLife Building in Midtown, Manhattan, shes already packing up the kitchen. Its 10:15 am on a Tuesday. She typically doesnt close until 11 am. These used to be her prime hours, but the pandemic has altered things dramatically for the food cart business. The banks are closed, the tourists are gone, and everyone is working from home. The Mama Jo of the carts namesake, real name Joanna Despas, tells me that shes been lucky to get more than 20 customers per day.
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="NefSde">
Despas is 70 years old, originally from rural northern Greece, and started up her breakfast cart 48 years ago. She cooks a blend of New York classics (bacon, egg, and cheese) and Mediterranean staples (spanakopita, olive bread) for the passing stream of professionals on their way to the office. Before Covid-19, Despas says, she routinely had lines around the block. These days, she mostly wheels her cart into town to feed the homeless for free, and to fire up the griddle for the handful of construction workers erecting a skyscraper across the street. Despas says shes made a negligible amount of money all year, and only shows up because it feels better to cook than to sit around at home all day.

<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="ldxzeA">
Despas holds out hope that eventually, when Broadway is open and the streets are full of tourists again, the street vending industry will bounce back. But shes pessimistic about the prospects of a total New York City recovery. Can Mama Jo, or any street vendor for that matter, count on an endless flow of hungry yuppies after theyve spent a year clocking in remotely? Is there going to be a sizable chunk of her clientele who simply wont go into the office as much? Is the Zoom revolution going to last past the vaccine? Nobody knows for sure.
</p>

We talked about how much she misses her regulars, and why, even after the seismic repercussions of the pandemic, shes still not planning on retiring anytime soon.

<strong>Who are your usual clients right now?</strong>
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="iO5lJz">
Right now, nobody. Everyone works from home. We have construction workers around here, because they are essential workers. But we dont have business, and I dont think well have business until the vaccines take hold. And even then, I dont know how much business will come back. A lot of businesses are closed, and when you look around the city, its dead. Vendors are gone. They pop up here and there, but they cant make any money out here. At this point, Im coming only for the homeless. There are so many of them, and where else are they going to get food?

<strong>So youre only out here to feed the homeless?</strong>

Yes, and I have a bunch of people who help me with that. The construction workers buy food for them. Theyre very nice. I have people who give me clothing and shoes that I hand out to them. Ive noticed a lot more homeless people out here lately. They were always there, but its never been like this. Theyre all over, and theres no food. All the vendors have been helping out. Some of them come up to me and say, “Theres a couple people down on Eighth Avenue that need this or that,” and I get it for them. You feel a little better. Youre doing something instead of staying home. Its good to be occupied.

<strong>Why do you think your business wont bounce back? </strong>
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="NVccKZ">
I talk to people who work in banks and they like what theyre doing. I think theyre going to come in for a couple days a week and work from home at other times. The residential people here have moved. I think thats how the city is going to work. Its not going to be easy for us. If you dont have a crowd for a year, youre not going to build it all back the next day. Its like starting back over from the beginning. Thats not restricted to vendors, the same goes for restaurants and delis.

<strong>How are you holding up financially?</strong>
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="EJvLVS">
No money. Were not making any money. Im doing okay. Im 70 years old and I get my Social Security. My grandmother taught me to always put away money for a rainy day. So Im okay, and my kids are okay and working from home. The big institutions are making money. The banks, the supermarkets, the franchises, theyre all doing okay. The people who are getting hurt are restaurants and vendors. Its different out in Brooklyn and Queens, you can see some more life out there. But certainly not out here.
</p>

<strong>How many customers are you seeing a day?</strong>
</p>

No more than 20.

<strong>And you used to have lines around the block, right?</strong>
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="yC6Qpj">
Oh yeah, and I dont know if thats ever going to happen again. But Im an optimistic person. Maybe if they open up the Broadway shows, well see what happens. But a lot of people have given up in this business. The garage used to have 200 pushcarts. Now weve got like, 100? Maybe less? And it gets less and less, because it gets more difficult.

<strong>Do you miss your regulars?</strong>

Of course. Ive got a lot of text messages from them. They leave messages on my social media. Ive got people from all over the world that know me, and they all ask how Im doing. They say, “I miss your food, I miss you.” They want to go back to normal. I see a lot of fear in people, and I dont want people to fear. Fear, worry, and stress kills you, completely.
</p>

<strong>Have you thought about moving the cart elsewhere?</strong>
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="JlIpzn">
At my age, Im not going to do that. Im feeding the homeless, and if Im not here I dont know where theyre going to eat. Im doing it for myself more than anything.
</p>

<strong>Has anything about this year made you want to retire?</strong>
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="RDUJEQ">
Im not going to retire. Im not that kind of person. Im never going to stay home, because I love what I do. I was only introduced to the idea of retirement in the United States. Its not as much of a thing in Greece. You see 80-year-olds working the fields out there. I put in 18 hours a day, and sleep like four hours at night. Im healthy. I cook everything myself. My diet is like the ancient Greeks — Ive been on this corner for 48 years, and I havent taken one sick day.

Thats the main takeaway from a <a href="https://docs.house.gov/meetings/IF/IF16/20210325/111407/HHRG-117-IF16-Wstate-ZuckerbergM-20210325-U1.pdf">statement he will provide to Congress on Thursday</a>, in a hearing about social medias role in spreading misinformation. But its also the mantra Zuckerberg and Facebook have been repeating for years, in targeted messaging like <a href="https://www.vox.com/2019/3/31/18289375/mark-zuckerberg-facebook-regulation-washington-post-op-ed">Washington Post op-eds</a> and paid ads aimed at the Beltway crowd.

But wed like someone else to take responsibility for …” and here you can fill in the blank, because it can range from anything from whether a <a href="https://www.vox.com/2016/9/9/12861634/facebook-photograph-vietnam-censorship">Pulitzer Prize-winning photo can run on the site</a> to whether <a href="https://www.vox.com/recode/22242627/trump-facebook-oversight-board">Donald Trump can post on Facebook</a>.
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="xRcv0S">
Now Facebook is in a position where everyone in Washington wants to do … something about Facebook, though exactly what depends on what part of the political spectrum they sit on. Republicans want Facebook to promise to stop censoring Republicans, though there isnt any evidence thats actually happening; Democrats want Facebook to promise not to destabilize democracy.
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="vqt8HH">
So now Zuckerberg is adding a twist to his standard request for regulation: He is telling Congress it should force Facebook — and everyone else who runs an internet platform — “to demonstrate that they have systems in place for identifying unlawful content and removing it.”
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="6icUv1">
Facebook wouldnt have to necessarily find all of that stuff and take down every last piece of it — Facebook is really big! But it would have to prove that it has spent a lot of time and money to <em>try</em> to do that.
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="dg95K8">
In return, Zuckerberg says, Facebook and everyone else who complies would get to keep the protections offered by <a href="https://www.vox.com/recode/2020/5/28/21273241/section-230-explained-trump-social-media-twitter-facebook">Section 230, a foundational piece of legislation</a> that lets online platforms host content uploaded by users without taking responsibility for that content.
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="9F0mn6">
On the one hand, this seems like a fairly straightforward proposition. After all, Facebook and other big platforms like YouTube and Twitter already have systems that allow them to police copyright violations on their property. Why shouldnt they have systems that do the same for “unlawful content”?
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="tnvQMv">
(Here its worth noting that in the early days of the platforms, their primary legal concern was avoiding the copyright claims that brought down Napster; the notion that the platforms might host content that could incite genocide or destabilize democracy wouldnt get much traction until a decade later.)
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="KXMWO9">
On the other hand, this isnt straightforward at all. Its more or less clear when something violates copyright. But it wont be at all clear what kind of content is “unlawful” — and waiting on Congress, which cant find any kind of bipartisan agreement on anything at all, to decide exactly what Facebook should allow on its properties means Facebook will be waiting a very long time to hear what those guidelines are.
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="g4bjNP">
Which, you might argue, is fine with Facebook, if you believe that Facebook merely wants to appear as though it wants to work with Congress and hope all of the momentum to regulate tech goes away someday.
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="cfu3mj">
A different but equally realpolitik take: Facebook figures theres going to be some kind of Section 230 reform, and by laying out a path it finds acceptable, it will have better odds of getting that result when it comes to negotiating with lawmakers and their staff. (Of note: Neither Alphabet CEO Sundar Pichai nor Twitter CEO Jack Dorsey, who are also virtually testifying at Thursdays hearing, asked Congress to modify Section 230 at all.)

<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="UNsrLz">
Critics will also point out that creating these kinds of rules and systems isnt nearly as big a problem for Facebook as it will be for smaller internet platform companies. (Remember that <a href="https://www.vox.com/recode/2019/7/24/20708359/facebook-ftc-settlement-criticism-5-billion-privacy-review-antitrust-mark-zuckerberg">Washington levied a $5 billion fine</a> and a new set of privacy guidelines on Facebook two years ago, and Facebook moved on without missing a beat because $5 billion isnt a lot of money to Facebook.) But since this isnt a new criticism, the company has a ready retort: Someone — not Facebook, certainly — should figure out the “definitions of an adequate system,” which “could be proportionate to platform size.”

<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="D5Fg0M">
Lets be clear: Facebook doesnt really want the government telling it what to do. It was happy(ish) to cut deals to <a href="https://www.vox.com/recode/2019/10/24/20929919/facebook-zuckerberg-murdoch-news-publishers-pay-content">pay Rupert Murdochs News Corp</a> for use of its content in America. In Australia, <a href="https://www.vox.com/recode/2021/2/20/22291425/facebook-australia-murdoch-law">Facebook threw a fit</a> when it was compelled to do the same thing by regulators there.
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="4pDcmP">
But what Facebook does want are legal guardrails and a promise that if it adheres to them, it can go about its very profitable business. Asking Congress to set those up — even if, or especially if, it takes a very long time — is a very small price to pay.

On Wednesday, the Senate voted to confirm former Pennsylvania Secretary of Health Dr. Rachel Levine as the new assistant secretary of health at the Department of Health and Human Services (HHS). Levines confirmation is historic: She is now the highest-ranking openly transgender government official in US history.

<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="PMLOyP">
In her new role, Levine will run the <a href="https://www.hhs.gov/ash/about-ash/organizational-chart/index.html">Office of the Assistant Secretary of Health (OASH)</a>, which oversees the nations public health policy. She will be a key administration figure as the White House tackles the deepening Covid-19 <a href="https://www.vox.com/coronavirus-covid19/">pandemic</a> that has claimed the lives of <a href="https://coronavirus.jhu.edu/map.html">more than 500,000 Americans</a>. But she will also play an important role in rolling back a host of Trump-era policies on reproductive, adolescent, and LGBTQ health. As a trained pediatrician with a history of supporting evidence-based adolescent health, and who has spoken about her own closeted trans youth, her confirmation marks a sea change from the right-wing <a href="https://equityfwd.org/rewire-abstinence-advocate-previously-cited-neglect-duty-tapped-create-hhs-strategies">political activists appointed to OASH</a> under former President Donald Trump.
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="CWAvwy">
Because of the historic nature of Levines appointment, there has been much talk about her trans identity. Meanwhile, her qualifications, which should not be overshadowed, have taken a back seat. For the last three years, shes been the secretary of health for Pennsylvania, where she has taken the lead on the states handling of the Covid-19 pandemic. Her clear-eyed press conferences instructing Pennsylvanians on how to survive the pandemic have earned praise from Democrats inside the state. Before she became the secretary of health, she was the states physician general.

<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="JHCNSZ">
Levine frequently takes a holistic approach to policy, <a href="https://www.phillymag.com/news/2020/07/16/rachel-levine-pennsylvania-health/">telling Philadelphia magazine</a> last July how she conceptualizes public health. “Economic opportunity is health. A living wage with an increase in the minimum wage is actually health,” she said. “Improving educational opportunities, improving nutrition, improving the environment, improving transportation for people is health. Getting rid of racism is health.”

<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="KByMs4">
Following President Joe Bidens nomination, Pennsylvania Gov. Tom Wolf called Levine “a highly skilled and valued member of my administration.”
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="iSatLn">
“She has been a wise and dedicated partner during this pandemic and throughout her career with the commonwealth,” <a href="https://www.governor.pa.gov/newsroom/gov-wolf-congratulates-thanks-dr-levine-following-nomination-by-president-elect-biden/">Wolf said in a statement</a>. “I couldnt be prouder of the tireless work shes done to serve Pennsylvanians and protect the public health.”
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="LdG8rR">
Levines confirmation would be important, if only because she would instantly become a recognizable and authoritative trans voice on public health at a moment when access to trans health care is increasingly threatened worldwide. With Biden taking an explicitly pro-trans stance, Levine steps into a visible role in charge of federal youth health policy.
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="ylA6Te">
“This marks a real turning point that honors and includes trans people,” Molly Bangs, director of Equity Forward, an HHS watchdog group, told Vox about Levines nomination in January. “Shes given every indication that she will continue to center equity from all perspectives when ascending to federal office. Transgender youth need real quality health care and access to information more so than ever. Shes demonstrated that that is very much a cornerstone of really what drives her career.”

Levine takes over a key federal public health office in a pivotal moment
</h3>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="DIFKjQ">
Her most immediate demand as assistant secretary of health is likely to be helping manage the federal governments handling of the pandemic.
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="mSLUlm">
But beyond the pandemic, she will take over a department that underwent some radical changes under Trump. The Office for Womens Health and the Office of Population Affairs oversee most of the governments reproductive health initiatives. The transition from Obama to Trump saw progressive initiatives tossed out in favor of anti-reproductive health policies, including rolling back the <a href="https://www.vox.com/21421292/trump-free-birth-control-iud-health-care">Obamacare birth control mandate</a>.
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="B2wiO7">
Meanwhile, the Office of Adolescent Health, which administered the Teen Pregnancy Prevention Program, was <a href="https://rewirenewsgroup.com/article/2019/04/04/the-trump-administration-wants-to-give-anti-choice-official-more-power-over-reproductive-health-programs/">shunted to a different offices oversight</a> and essentially shuttered under Trump. One of Levines first accomplishments in Pennsylvania was establishing the states Office of Adolescent Health, and shell now be tasked with restoring that office at the federal level

<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="stwkVt">
“This position of assistant secretary of health has a massive portfolio of issues,” said Bangs. “Dr. Levine and her colleagues will have their work cut out for them, and they will need a really proactive agenda in order to not just reverse the damage thats been done but also protect and expand LGBTQ rights [and] sexual and reproductive health [access].”

That work will get underway almost immediately. Even before Levines confirmation hearing was scheduled, the Biden administration took initial steps to overturn Trump policy at OASH. The White House has already launched a review of the <a href="https://www.vox.com/policy-and-politics/2019/2/22/18236227/abortion-planned-parenthood-gag-rule-title-x">Title X domestic gag rule</a> — which banned federal funding to health clinics that perform or refer patients for abortions.

Levines office will oversee the review, putting her at the center of US reproductive health policy for the next several years.

Levine will not be the first openly trans government official
</h3>

Though she is deeply qualified for the position, Levines trans identity has grabbed the lions share of media attention.

<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="K29J7Z">
Newspapers and websites have trumpeted her identity, almost to the exclusion of any other facts about her. “Biden picks 1st transgender person for Senate-confirmed post,” read an <a href="https://apnews.com/article/rachel-levine-health-secretary-4eee53439e9c2b4c27fcf4e7f572cb0e">Associated Press headline</a> in January. Even the LGBTQ Nations <a href="https://www.lgbtqnation.com/2021/01/joe-biden-picks-transgender-woman-assistant-health-secretary/">headline</a> — “Joe Biden picks transgender woman for assistant health secretary” — did not mention Levine by name.
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="q5agjd">
While Levine is now the highest-ranking transgender official in the federal government, a welcome step forward for a community that has historically had very little institutional power, she is not the first openly trans government official appointed by a president; she is just the first to be confirmed by the Senate. President Barack Obama named trans woman <a href="https://abcnews.go.com/Politics/amanda-simpson-transgender-presidential-appointee-begins-work-commerce/story?id=9477161">Amanda Simpson</a> senior technical adviser in the Commerce Departments Bureau of Industry and Security in 2010, though it was not a Senate-confirmed appointment.
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="ZEem2y">
Levines trans identity will inform her new role as one of the nations top public health officials.<strong> </strong>“At a time when access to health care is a growing crisis for transgender people made worse by anti-LGBTQ legislation and legislators across the nation, Dr. Levine has the empathy to understand the health needs of our diverse country and the skillset to improve them,” Alphonso David, president of the Human Rights Campaign, has <a href="https://www.hrc.org/press-releases/human-rights-campaign-on-the-historic-nomination-of-dr-dr-rachel-levine-to-be-assistant-health-secretary">said in a statement</a>.

Levine has spoken previously about her experiences as a closeted trans child. At a conference in 2015, just after shed been appointed Pennsylvanias acting physician general, she described growing up playing football and hockey near Boston in the 60s, but also carrying with her a deep secret. ”All I knew is I wanted to be a girl, or I was a girl, or female,” <a href="https://www.pennlive.com/midstate/2015/03/transgender_conference_rachel.html">she told a crowd</a> in Swatara Township, Pennsylvania.
</p>
<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom" id="UxTu5M">
During that conference, Levine spoke directly to the states trans youth. “What I want to tell those kids is I am there for you. We are here for you,” she said. “Please dont harm yourself and please dont despair, because we are there for you.”

Its a message shes continued to stand by throughout her public health career, even as critics and random internet trolls relentlessly attacked her gender identity and appearance. And shes making history in a time and place when trans “firsts” are becoming rarer — an appointment that is as much a win for trans people as it is for a well-qualified doctor with a lifetime of public health experience.
