+ + + ++Type III interferons have been touted as promising therapeutics in outpatients with coronavirus disease 2019 (COVID-19). We conducted a randomized placebo-controlled trial in 120 patients with mild to moderate COVID-19 to determine whether a single, 180 mcg subcutaneous dose of Peginterferon Lambda-1a (Lambda) could shorten the duration of viral shedding (primary endpoint) or symptoms (secondary endpoint, NCT04331899). In both the 60 patients receiving Lambda and the 60 receiving placebo, the median time to cessation of viral shedding was 7 days (hazard ratio [HR] = 0.81; 95% confidence interval [CI] 0.56 to 1.19). Symptoms resolved in 8 and 9 days in Lambda and placebo, respectively (HR 0.94; 95% CI 0.64 to 1.39). At enrollment; 41% of subjects were SARS-CoV-2 IgG seropositive; compared to placebo, lambda tended to delay shedding cessation in seronegatives (aHR 0.66, 95% CI 0.39-1.10) and to hasten shedding cessation in seropositives (aHR 1.58, 95% CI 0.88-2.86; p for interaction = 0.03). Liver transaminase elevations were more common in the Lambda vs. placebo arm (15/60 vs 5/60; p = 0.027). In this study, a single dose of subcutaneous Peginterferon Lambda-1a neither shortened the duration of SARS-CoV-2 viral shedding nor improved symptoms in outpatients with uncomplicated COVID-19. +
++Our trust has an urgent need to make short-term (3-4 days in advance) informed operational decisions which take into account best-practice treatment regimens and known clinical features of COVID19 inpatients. We believe that any model which is relied upon for operational decision making should have clinically identifiable parameters. Our model9s parameters take into account the conversion rates from acute wards into wards equipped with Non-Invasive Ventilation (NIV) and Mechanical Ventilation (MV), the typical time that these conversions take place and, the historical non-COVID usage of NIV and MV beds. We have observed that this clinical performance is mathematically identical to a series of linear delays on the time varying inpatient level. High frequency inpatient data, sampled ~4 hourly, has allowed our hospital trust to predict total critical care usage up to 4 days in advance without making any assumptions on upcoming inpatients. It is based entirely upon current bed occupancy levels and measured clinical pathways. Through back-testing over the recent 4 months, the bounds of this model include 93.8% of all 4 day inpatient sequences. The average next-day error is 0.8 (95% CI: 0.44, 1.15) and so the system tends to over-predict the next day critical care inpatients by approximately 1 bed. Potential extensions to the basic model include adjustments for seasonality, case mix, probabilistic marginalisation and known discharges. +
++Background. Antigen-detecting rapid diagnostic tests for SARS-CoV-2 offer new opportunities for the quick and laboratory-independent identification of infected individuals for control of the SARS-CoV-2 pandemic. Methods. We performed a prospective, single-center, point of care validation of two antigen-detecting rapid diagnostic tests (Ag-RDT) in comparison to RT-PCR on nasopharyngeal swabs. Findings. Between October 9th and 23rd, 2020, 1064 participants were enrolled. The PanbioTM Covid-19 Ag Rapid Test device (Abbott) was validated in 535 participants, with 106 positive Ag-RDT results out of 124 positive RT-PCR individuals, yielding a sensitivity of 85.5% (95% CI: 78.0-91.2). Specificity was 100.0% (95% CI: 99.1-100) in 411 RT-PCR negative individuals. The Standard Q Ag-RDT (SD Biosensor, Roche) was validated in 529 participants, with 170 positive Ag-RDT results out of 191 positive RT-PCR individuals, yielding a sensitivity of 89.0% (95%CI: 83.7-93.1). One false positive result was obtained in 338 RT-PCR negative individuals, yielding a specificity of 99.7% (95%CI: 98.4-100). For individuals presenting with fever 1-5 days post symptom onset, combined Ag-RDT sensitivity was above 95%. Interpretation. We provide an independent validation of two widely available commercial Ag-RDTs, both meeting WHO criteria of at least 80% sensitivity and at least 97% specificity. Although less sensitive than RT-PCR, these assays could be beneficial due to their rapid results, ease of use, and independence from existing laboratory structures. Testing criteria focusing on patients with typical symptoms in their early symptomatic period onset could further increase diagnostic value. +
++Drug repurposing provides a rapid approach to meet the urgent need for therapeutics to address COVID-19. To identify therapeutic targets relevant to COVID-19, we conducted Mendelian randomization (MR) analyses, deriving genetic instruments based on transcriptomic and proteomic data for 1,263 actionable proteins that are targeted by approved drugs or in clinical phase of drug development. Using summary statistics from the Host Genetics Initiative and the Million Veteran Program, we studied 7,554 patients hospitalized with COVID-19 and >1 million controls. We found significant Mendelian randomization results for three proteins (ACE2: P=1.6x10^-6, IFNAR2: P=9.8x10^-11, and IL-10RB: P=1.9x10^-14) using cis-eQTL genetic instruments that also had strong evidence for colocalization with COVID-19 hospitalization. To disentangle the shared eQTL signal for IL10RB and IFNAR2, we conducted phenome-wide association scans and pathway enrichment analysis, which suggested that IFNAR2 is more likely to play a role in COVID-19 hospitalization. Our findings prioritize trials of drugs targeting IFNAR2 and ACE2 for early management of COVID-19. +
++ABSTRACT Clinical laboratories of the developing world are overwhelmed with RT PCR SARS Cov2 testing demands. It is of paramount importance that each clinical laboratory use an appropriate cutoff value in the interpretation of SARS Cov2 realtime RT PCR results, which is specific to their laboratory performances as ISO 15189 recommendations stipulate. We applied an indirect statistical method to a large mixed data set of Ct values (ORF1ab and N) to estimate cut-off Ct value (~32 cycles).we conclude that the use of indirect statistical approaches to estimate cut-off value in the interpretation of SARS-Cov2 real-time RT PCR results may improve differential diagnosis of COVID 19 cases with low risk of infectivity, and may help to better estimates of the burden of COVID 19 disease. +
++Abstract: An improved understanding of the immunity offered by the antibodies developed against SARS-CoV-2 is critical for the development of diagnostic tests and vaccines. Our study aimed at the longitudinal analysis of antibody persistence and its dynamic trend over a period of eight months in a group of COVID-19 recovered patients who tested positive by real-time quantitative PCR for SARS-CoV-2 in the period between the 18th and 30th of March, 2020. The subjects were divided into two groups based on disease severity: mild and moderately-severe. The MAGLUMI 2019-nCoV lgM/lgG chemiluminescent analytical system (CLIA) assay was used to analyse the antibody titres. Robust IgG antibody persistency was demonstrated in 76.7 % of the subjects (23 out of 30) at eight months post infection. The results of this study highlight an important point in terms of the association between humoral immune response and disease severity. Patients who might have experienced a relatively moderate-severe infection may develop a robust immunity that could persist for a longer duration. +
++Background: We analyzed antibody response patterns according to level of disease severity in patients with novel coronavirus disease 2019 (COVID-19) in Japan. Methods: We analyzed 611 serum specimens from 231 patients with COVID-19 (mild, 170; severe, 31; critical, 30). IgM and IgG antibodies against nucleocapsid protein (N) and spike 1 protein (S1) were detected by enzyme-linked immunosorbent assays. Findings: The peaks of fitting curves for the OD values of IgM and IgG antibodies against N appeared simultaneously, while those against S1 were delayed compared with N. The OD values of IgM against N and IgG against both N and S1 were significantly higher in the severe and critical cases than in the mild cases at 11 days after symptom onset. The seroconversion rates of IgG were higher than those of IgM against both N and S1 during the clinical course based on the optimal cut-off values defined in this study. The seroconversion rates of IgG and IgM against N and S1 were higher in the severe and critical cases than in the mild cases. Conclusion: Our findings show that a stronger antibody response occurred in COVID-19 patients with greater disease severity and there were low seroconversion rates of antibodies against N and S1 in the mild cases. The antibody response patterns in our population suggest a second infection pattern, leading us to hypothesize that cross-reactivity occurs between SARS-CoV-2 and past infection with other human coronaviruses. +
++As predicting the trajectory of COVID-19 disease is challenging, machine learning models could assist physicians determine high-risk individuals. This study compares the performance of 18 machine learning algorithms for predicting ICU admission and mortality among COVID-19 patients. Using COVID-19 patient data from the Mass General Brigham (MGB) healthcare database, we developed and internally validated models using patients presenting to Emergency Department (ED) between March-April 2020 (n = 1144) and externally validated them using those individuals who encountered ED between May-August 2020 (n = 334). We show that ensemble-based models perform better than other model types at predicting both 5-day ICU admission and 28-day mortality from COVID-19. CRP, LDH, and procalcitonin levels were important for ICU admission models whereas eGFR <60 ml/min/1.73m2, ventilator use, and potassium levels were the most important variables for predicting mortality. Implementing such models would help in clinical decision-making for future COVID-19 and other infectious disease outbreaks. +
++Background: Wastewater-based epidemiology (WBE) is one of the most promising approaches to effectively monitor the spread of the novel coronavirus disease 2019 (COVID-19). The virus concentration in faeces and its temporal variations are essential information for WBE. While some clinical studies have reported severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) concentrations in faeces, the value varies amongst patients and changes over time. Aim: The present study aimed to examine how the temporal variations in the concentration of virus in faeces affect the monitoring of disease incidence. We re-analysed the experimental findings of clinical studies to estimate the duration of virus shedding and the faecal virus concentration. Method: Available experimental data as of 23 October, 2020 were collected and patient data reported in Germany were included for further analysis. The viral shedding kinetics was modelled, and the dynamic model was fitted to the collected experimental data by a Bayesian framework. Using samples of posterior distributions, the duration of viral shedding and the concentration of virus copies in faeces over time were computed. Results: We estimated the median concentration of SARS-CoV-2 in faeces as 2.6 (95% Credible Interval (CrI): 0.22-4.8) log copies per gram (g) of faeces over the shedding period, and our model implied that the duration of viral shedding was 23.2 days (95% CrI: 19.5-31.5), given the current standard quantification limit (Ct = 40). With simulated incidences, our results also indicated that a one-week delay between symptom onset and wastewater sampling increased the estimation of incidence by 13.5%. Conclusions: Our results demonstrated that the temporal variation in virus concentration in faeces affects microbial monitoring systems such as WBE. The present study also implied the need for adjusting the estimates of virus concentration in faeces by incorporating the kinetics of unobserved concentrations. The method used in this study is easily implemented in further simulations; therefore, the results of this study might contribute to enhancing disease surveillance and risk assessments that require quantities of virus to be excreted into the environment. +
++Introduction: Due to the spreading of COVID-19, one key challenge was to reduce potential staff shortages in the healthcare sector. Besides a reactivation of retired healthcare workers, medical students were considered to support this task. We herein analysed the commitment of medical students during the COVID-19-pandemic as well as their burdens and anxieties. Methods: Via an online survey, medical students in Germany were asked regarding these themes. The survey period took place during a two-week period in April and Mai 2020. 1241 questionnairs were included in the analysis. Results: 67.9% [65.3; 70.5] of the participants reported that they volunteered during the pandemic. Simultaneously, 88.9% [86.9; 90.5] of the participants stated to be against a compulsory recruitment in this context. Students who volunteered showed a significantly lower anxiety index than students, who did not volunteer. Concerns about transferring the virus to patients or relatives were significantly higher than concerns about the students own infection. Conclusion: The results of this survey study suggest that the majority of students are working voluntary during the pandemic. They declared mostly that they had adequate opportunities to protect themselves. Finally, their biggest fear was to infect other patients, relatives, or friends. +
++We propose an original model based on a set of coupled delay differential equations with fourteen delays in order to accurately estimate the incubation period of COVID-19, employing publicly available data of confirmed corona cases. In this goal, we separate the total cases into fourteen groups for the corresponding fourteen incubation periods. The estimated mean incubation period we obtain is 6.74 days (95% Confidence Interval(CI): 6.35 to 7.13), and the 90th percentile is 11.64 days (95% CI: 11.22 to 12.17), corresponding to a good agreement with statistical supported studies. This model provides an almost zero-cost approach to estimate the incubation period. +
++Approximately 2.8 billion people rely on polluting cooking fuels (e.g. wood, kerosene), exposing them to health-damaging household air pollution. A key access barrier to clean cooking fuels (e.g. liquefied petroleum gas) is affordability. By enabling households to pay in small increments, pay-as-you-go (PAYG) liquefied petroleum gas (LPG) could help promote clean cooking, and support continued LPG use through periods of economic downturn. We investigate the ability of PAYG LPG to sustain access to clean cooking from January 2018- June 2020, including during COVID-19 lockdown (March-June 2020) in an informal settlement in Nairobi, Kenya. We utilize novel PAYG LPG smart meter data to document cooking/spending patterns from 426 PAYG LPG customers and semi-structured interviews among a subset of seven households. Objective cooking pattern comparisons are made to those cooking with full 6kg cylinder LPG and polluting fuel users from 23 households in peri-urban Eldoret in western Kenya, using stove monitoring data. Average PAYG LPG consumption was 0.97 kg/capita/month (11.6 kg/capita/year) prior to COVID-19 lockdown. Despite adverse economic impacts of the lockdown, 95% of households continued using PAYG LPG, and consumption increased to 1.22 kg/capita/month (March-June 2020). Daily cooking events using PAYG LPG increased by 60% (1.07 events/day (pre-lockdown) to 1.72 events/day (lockdown)). In contrast, among seven households purchasing full 6kg cylinder LPG in Eldoret, average days/month using LPG declined by 75% (17 to four days) during COVID-19 lockdown. Median PAYG LPG payment frequency doubled (from every 8 days to every 4 days) during lockdown, while average payment amount was nearly halved (336 Kenyan Shillings (KSh)/US$3.08 to 179 KSh/US$1.64). Interviewed customers reported numerous benefits of PAYG LPG beyond fuel affordability, including safety, time savings, cylinder delivery and user-friendliness. PAYG LPG helped sustain clean cooking during COVID-19 lockdown, possibly averting increases in polluting cooking fuel use and associated household air pollution exposures. +
++Background: As part of on-going efforts to contain the COVID-19 pandemic, understanding the role of asymptomatic patients in the transmission system is essential to infection control. However, optimal approach to risk assessment and management of asymptomatic cases remains unclear. Methods: This study involved a SEINRHD epidemic propagation model, constructed based on epidemiological characteristics of COVID-19 in China, accounting for the heterogeneity of social network. We assessed epidemic control measures for asymptomatic cases on three dimensions. Impact of asymptomatic cases on epidemic propagation was examined based on the effective reproduction number, abnormally high transmission events, and type and structure of transmission. Results: Management of asymptomatic cases can help flatten the infection curve. Tracking 75% of asymptomatic cases corresponds to an overall reduction in new cases by 34.3% (compared to tracking no asymptomatic cases). Regardless of population-wide measures, family transmission is higher than other types of transmission, accounting for an estimated 50% of all cases. Conclusions: Asymptomatic case tracking has significant effect on epidemic progression. When timely and strong measures are taken for symptomatic cases, the overall epidemic is not sensitive to the implementation time of the measures for asymptomatic cases. +
++COVID-19 is a highly infectious disease. Studies suggest that its severity is amplified in patients diagnosed with Diabetes Mellitus. In this study, the correlation between the prevalence of COVID-19 and Diabetes was analyzed at the regional and global scale using data extracted from WHO and IDF Diabetes Atlas. For the regional investigation data was assorted into ten regions including Central Asia, Middle east and western Asia, Africa, North America and the Caribbean, South east Asia, East Asia, Europe, South and Central America, South Asia and Oceania. The results show a positive correlation coefficient of 0.47 in Middle east and western Asia. While at the global scale analysis all the selected countries were considered together and a correlation coefficient of 0.32 was observed. This number was increased to 0.69 when the top most affected countries by COVID-19 were considered for the analysis. In order to investigate the time dependent relationship of the two diseases, the data was analysed in five windows of 45 days each since the beginning of pandemic. The results show an increasing pattern of the correlation coefficient in the last three windows. Overall, based on this study by increasing the prevalence of Diabetes Mellitus, the prevalence of COVID-19 cases may also increase. +
++Unemployment shocks from the COVID-19 pandemic have reignited concerns over the long-term effects of job loss on population health. Past research has highlighted the corrosive effects of unemployment on health and health behaviors. This study examines whether the effects of job loss on changes in body mass index (BMI) are moderated by genetic predisposition using data from the U.S. Health and Retirement Study (HRS). To improve detection of gene-by-environment (G x E) interplay, we interacted layoffs from business closures (a plausibly exogenous environmental exposure) with whole-genome polygenic scores (PGSs) that capture genetic contributions to both the population mean (mPGS) and variance (vPGS) of BMI. Results show evidence of genetic moderation using a vPGS (as opposed to an mPGS) and indicate genome-wide summary measures of phenotypic plasticity may further our understanding of how environmental stimuli modify the distribution of complex traits in a population. +
