<li><strong>ARDSFlag: An NLP/Machine Learning Algorithm to Visualize and Detect High-Probability ARDS Admissions Independent of Provider Recognition and Billing Codes</strong> -

<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom">
Acute respiratory distress syndrome (ARDS) is a type of respiratory failure characterized by bilateral pulmonary infiltrates that cannot be explained entirely by cardiogenic pulmonary edema. ARDS is the primary cause of mortality in COVID-19 patients and one of the leading causes of morbidity and mortality in ICUs. Despite its significance and prevalence, the detection of ARDS remains highly variable and inconsistent. In this work, we develop a tool to automate the diagnosis of ARDS based on the Berlin definition to increase the accuracy of ARDS detection using electronic health record (EHR) fields. ARDSFlag applies machine learning (ML) and natural language processing (NLP) techniques to evaluate Berlin criteria by incorporating structured and unstructured data. The output is the ARDS diagnosis, onset time, and severity. We have also developed a visualization that helps clinicians efficiently assess ARDS criteria retrospectively and in real time. The method includes separate text classifiers trained using large training sets to find evidence of bilateral infiltrates in radiology reports (accuracy of 91.9%+-0.5%) and heart failure/fluid overload in radiology reports (accuracy 86.1%+-0.5%) and echocardiogram notes (accuracy 98.4%+-0.3%). A holdout set of 300 cases, which was blindly and independently labeled for ARDS by two groups of clinicians, shows that the algorithm generates an overall accuracy of 89.0%, with a specificity of 91.7%, recall of 80.3%, and precision of 75.0%. Compared with two other ARDS identification methods used in the literature, ARDSFlag shows higher performance in all accuracy measures (an increase of 25.5% in overall accuracy, 6.5% in specificity, 44.2% in recall, 31.7% in precision, and 38.20% in F1-score over the best of the two detection methods).

<li><strong>Comparative adverse effects, perceptions and attitudes related to BNT162b2, mRNA1273, or JNJ-78436735 SARS-CoV-2 vaccines: A population-based longitudinal cohort</strong> -

Importance: Long-term control of SARS-CoV-2 requires effective vaccination strategies. This has been challenged by public mistrust and spread of misinformation regarding vaccine safety. Hence, better understanding and communication on the longer-term and comparative experiences of general population individuals following SARS-CoV-2 vaccination are required. Objective: To evaluate and compare self-reported adverse effects following SARS-CoV-2 vaccination, participants perceptions regarding vaccinations and their compliance with recommended public health measures. Design, Setting and Participants: Population-based longitudinal cohort of 575 adults, randomly selected from all individuals presenting to the reference vaccination center of the Canton of Zurich, Switzerland, for receipt of BNT162b2, mRNA1273, or JNJ-78436735. Exposures: BNT162b2, mRNA1273, or JNJ-78436735 vaccines. Main Outcomes and Measures: Primary outcomes included period prevalence, onset, duration, and severity of self-reported adverse effects over 12 weeks following vaccination with a specific focus on the proportion of participants reporting allergic reactions, menstrual irregularities, or cardiac adverse effects, or requiring hospitalization. Secondary outcomes included risk factors associated with reporting adverse effects, perception of vaccine importance, trust in public health authorities and pharmaceutical companies, and compliance with recommended public health measures. Results: 454 (79.0%) participants reported at least one adverse effect during 12 weeks after vaccination. Prevalence was highest among mRNA-1273 recipients (88.7% vs. 77.3% after BNT162b2, 69.1% after JNJ-78436735). Most adverse effects were systemic (72%), occurred within 24 hours (67.9%), and resolved in less than three days (76.3%). 85.2% were reported as mild or moderate. Allergic reactions were reported by 0.3% of participants, hospitalizations by 0.7%, cardiac adverse effects by 1.4%. Menstrual irregularities were reported by 9% of female participants younger than 50 years. Female sex, younger age, higher education, and receipt of mRNA-1273 were associated with reporting adverse effects. Compared to JNJ-78436735 recipients, a higher proportion of mRNA vaccine recipients agreed that vaccination is important (87.5% vs. 28.5%), and trusted public health authorities (80.2% vs. 30.3%) and pharmaceutical companies (71.7% vs. 23.6%). Conclusions and Relevance: Our population-based cohort provided real-world data on self-reported adverse effects following SARS-CoV-2 vaccination and highlights the importance of transparent communication regarding adverse effects and building trust in public health authorities to ensure successful future vaccination campaigns.

<li><strong>Impact of Differential Vaccine Effectiveness on COVID-19 Hospitalization Cases: Projections for 10 Developed Countries where Booster Vaccines were Recommended</strong> -

Background &amp; Objectives: In a previous analysis, a decision-analytic model was used to analyze the clinical and economic impact of the differences in effectiveness between the two licensed mRNA COVID-19 booster vaccines, mRNA-1273 and BNT162b2, in 2022 for adults aged 18 years and older in the United States (US). In this analysis, the same model was used to estimate the impact that administering first booster doses with mRNA-1273 could have had on COVID-related hospitalizations and costs over a 6-month period in 10 developed countries (Australia, Canada, France, Germany, Italy, Japan, South Korea, Spain, United Kingdom [UK], and US), considering updated effectiveness data. Methods: The model was used to estimate number of hospitalizations and related costs using the actual vaccine distribution for the first COVID-19 booster from each country. These estimates were compared to a scenario where 100% of doses for that 6-month period was assumed to be mRNA-1273. The effectiveness of mRNA-1273 compared to BNT162b2 was estimated from real world data from the UK. Results: The total number of doses switched to the mRNA-1273 booster would range from 4.3 million in Spain to 39.4 million in Japan. The number of hospitalizations and associated hospitalization costs would be expected to fall in all countries, with the proportional decrease ranging from 1.1% (16,800 fewer) in Germany to 8.8% (25,100 fewer) in Australia. Conclusions: Real-world effectiveness data suggest that a booster dose of the mRNA-1273 vaccine may be more effective compared to other vaccines used for booster doses. Given this difference in effectiveness, results of this analysis demonstrate that switching to 100% mRNA-1273 boosters would have reduced the number of hospitalizations and associated costs in each country during the first 6 months of the omicron period.

<li><strong>Nation-wide mammography screening participation in Denmark during the COVID-19 pandemic: An observational study</strong> -

Background&lt;br /&gt;In most of the world, the mammography screening programmes were paused at the start of the pandemic, whilst mammography screening continued in Denmark. We examined the mammography screening participation during the COVID-19 pandemic in Denmark.&lt;br /&gt;Methods&lt;br /&gt;The study population comprised all women aged 50-69 years old invited to participate in mammography screening from 2016-2021 in Denmark based on data from the Danish Quality Database for Mammography Screening in combination with population-based registries. Using a generalised linear model, we estimated prevalence ratios (PR) and 95% confidence intervals (CI) of mammography screening participation within 90, 180 and 365 days since invitation during the pandemic in comparison with the previous years adjusting for age, year and month of invitation.&lt;br /&gt;Results&lt;br /&gt;The study comprised 1,828,791 invitations among 847,766 women. Before the pandemic, 80.2% of invitations resulted in participation in mammography screening within 90 days, 82.7% within 180 days and 83.1% within 365 days. At the start of the pandemic, the participation in screening within 90 days was reduced to 69.9% for those invited in pre-lockdown and to 76.5% for those invited in 1st lockdown. Extending the length of follow-up time to 365 days only a minor overall reduction was observed (PR=0.94; 95% CI: 0.93-0.95 in pre-lockdown and PR=0.97; 95% CI: 0.96-0.97 in 1st lockdown). A lower participation was; however, seen among immigrants and among women with a low income.&lt;br /&gt;Conclusions&lt;br /&gt;The short-term participation in mammography screening was reduced at the start of the pandemic, whilst only a minor reduction in the overall participation was observed with longer follow-up time indicating that women postponed screening. Some groups of women; nonetheless, had a lower participation indicating that the social inequity in screening participation was exacerbated during the pandemic.&lt;br /&gt;Funding&lt;br /&gt;The study was funded by the Danish Cancer Society Scientific Committee (grant number R321-A17417) and the Danish regions.

<li><strong>Clinical Subphenotypes of Multisystem Inflammatory Syndrome in Children: An EHR-based cohort study from the RECOVER program</strong> -

Background: Multi-system inflammatory syndrome in children (MIS-C) represents one of the most severe post-acute sequelae of SARS-CoV-2 infection in children, and there is a critical need to characterize its disease patterns for improved recognition and management. Our objective was to characterize subphenotypes of MIS-C based on presentation, demographics and laboratory parameters. Methods: We conducted a retrospective cohort study of children with MIS-C from March 1, 2020 - April 30, 2022 and cared for in 8 pediatric medical centers that participate in PEDSnet. We included demographics, symptoms, conditions, laboratory values, medications and outcomes (ICU admission, death), and grouped variables into eight categories according to organ system involvement. We used a heterogeneity-adaptive latent class analysis model to identify three clinically-relevant subphenotypes. We further characterized the sociodemographic and clinical characteristics of each subphenotype, and evaluated their temporal patterns. Findings: We identified 1186 children hospitalized with MIS-C. The highest proportion of children (44.4%) were aged between 5-11 years, with a male predominance (61.0%), and non-Hispanic white ethnicity (40.2%). Most (67.8%) children did not have a chronic condition. Class 1 represented children with a severe clinical phenotype, with 72.5% admitted to the ICU, higher inflammatory markers, hypotension/shock/dehydration, cardiac involvement, acute kidney injury and respiratory involvement. Class 2 represented a moderate presentation, with 4-6 organ systems involved, and some overlapping features with acute COVID-19. Class 3 represented a mild presentation, with fewer organ systems involved, lower CRP, troponin values and less cardiac involvement. Class 1 initially represented 51.1% of children early in the pandemic, which decreased to 33.9% from the pre-delta period to the omicron period. Interpretation: MIS-C has a spectrum of clinical severity, with degree of laboratory abnormalities rather than the number of organ systems involved providing more useful indicators of severity. The proportion of severe/critical MIS-C decreased over time.

The aim of this paper is to model COVID-19 based on Markov chains. First, we introduce basic concepts of Markov chains with examples from different disciplines. Second, we use different types of Markov chains to model COVID-19, including confirmed cases, death and recovered cases and forecasting future confirmed cases. Third, we give conclusions based on these models and ideas for future work.

Historically SARS-CoV-2 secondary attack rates (SAR) have been based on PCR positivity on screening symptomatic contacts, this misses transmission events and identifies only symptomatic contacts who are PCR positive at the time of sampling. We used serology to detect the relative transmissibility of Alpha Variant of Concern (VOC) to non-VOC SARS-CoV-2 to calculate household secondary attack rates. We identified index patients diagnosed with Alpha and non-VOC SARS-CoV-2 across two London Hospitals between November 2020 and January 2021 during a prolonged and well adhered national lockdown. We completed a household seroprevalence survey and found that 61.8% of non-VOC exposed household contacts were seropositive compared to 82.1% of Alpha exposed household contacts. The odds of infection doubled with exposure to an index diagnosed with Alpha. There was evidence of transmission events in almost all households. Our data strongly support that estimates of SAR should include serological data to improve accuracy and understanding.

Background The Covid Collab study was a citizen science mobile health research project set up in June 2020 to monitor COVID-19 symptoms and mental health through questionnaire self-reports and passive wearable device data. Methods Using mobile health data, we consider whether a participant is suffering from long COVID in two ways. Firstly, by whether the participant has a persistent change in a physiological signal commencing at a diagnosis of COVID-19 that last for at least twelve weeks. Secondly, by whether a participant has self-reported persistent symptoms for at least twelve weeks. We assess sociodemographic and wearable-based risk factors for the development of long COVID according to the above two categorisations. Findings Persistent changes to physiological signals measured by com- mercial fitness wearables, including heart rate, sleep, and activity, are visible following a COVID-19 infection and may help differentiate people who develop long COVID. Anxiety and depression are significantly and persistently affected at a group level following a COVID-19 infection. We found the level of activity undertaken in the year prior to illness was protective against long COVID and that symptoms of depression before and during the acute illness may be a risk factor. Interpretation Mobile health and wearable devices may prove to be a useful resource for tracking recovery and presence of long-term sequelae to COVID-19. Mental wellbeing is significantly negatively effected on average for an extended period of time following a COVID-19 infection.

The measurement of CO2 has positioned itself as a low-cost and straightforward technique to indirectly control indoor air quality, allowing the reduction of the concentration of potentially pathogen-loaded aerosols to which we are exposed. However, on numerous occasions, bad practice limits the technique for CO2 level interpreting and does not apply methodologies that guarantee air renewal. This work proposes a new methodology for measuring and controlling CO2 levels for indoor air in shared spaces. The proposed methodology is based on three stages: diagnosis, correction protocols, and monitoring/control/surveillance (MCS). The procedure is explained using a cultural center as an actual base case study. Additionally, the procedure was validated by implementing 40 voluntary commercial spaces in Zaragoza (Spain). Standardization of methods is suggested so that the measurement of CO2 becomes an effective strategy to control the airborne transmission of pathogens and thus prevent future Covid-19 outbreaks and novel pandemics.

The clinical course and severity of COVID-19 vary among patients. This study aimed to investigate the association of the interferon receptor ( IFNAR2 ) rs2236757 and oligoadenylate synthetase 3 ( OAS3 ) rs10735079 gene polymorphisms with risk of COVID-19 infection and severity among Palestinian patients.  The study was conducted between April and May 2021 on 154 participants that divided into three groups: the control group (RT-PCR-negative, n=52), the community cases group (RT-PCR-positive, n= 70) and the critically ill cases (ICU group; n=32). Genotyping of the studied polymorphisms was conducted by amplicon-based next-generation sequencing. The genotype distribution of the IFNAR2 rs2236757 was significantly different among the study groups (P = 0.001), while no significant differences were observed in the distribution of OAS3 rs10735079 genotypes (P = 0.091). Logistic regression analysis adjusted for possible confounding factors revealed a significant association between the risk allele rs2236757A and critical COVID-19 illness (P &lt; 0.025). Among all patients, the rs2236757GA carriers were more likely to have sore throat (OR, 2.52 (95% CI 1.02-6.24); P = 0.011); the risk allele rs2236757A was associated with dyspnea (OR, 4.70 (95% CI 1.80-12.27); P &lt; 0.001), while the rs10735079A carriers were less prone to develop muscle aches (OR, 0.34  (95% CI 0.13-0.88); P = 0.0248) and sore throat (OR, 0.17 (95% CI 0.05-0.55); P &lt; 0.001). In conclusion, our results revealed that the rs2236757A variant was associated with critical COVID-19 illness and dyspnea, whereas the rs10735079A variant was protective for muscle aches and sore throat.

<li><strong>Impact of the COVID-19 restrictions on the epidemiology of Cryptosporidium spp. in England and Wales, 2015-2021. A time-series analysis</strong> -

<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom">
<b>Background</b> In England and Wales, cryptosporidiosis cases peak in spring and autumn, usually associated with zoonotic and environmental exposures (<i>Cryptosporidium parvum</i>, spring/autumn) and with overseas travel and water-based activities (<i>Cryptosporidium hominis</i>, autumn). Restrictions to control the COVID-19 pandemic prevented social mixing and access to swimming pools and restaurants for many months. Foreign travel from the UK also reduced by 74% in 2020. However, these restrictions potentially increased environmental exposures as people sought alternative countryside activities locally. To inform and strengthen surveillance programmes, we investigated the impact of COVID-19 restrictions on the epidemiology of <i>C. hominis</i> and <i>C. parvum cases</i>. <b>Methods</b><i>Cryptosporidium</i>-positive stools, with case demographic data, are referred routinely for genotyping to the national Cryptosporidium Reference Unit (CRU). Cases were extracted from the CRU database (01 January 2015 to 31 December 2021). We defined two periods for pre- and post-COVID-19 restrictions implementation corresponding to the first UK-wide lockdown on 23 March 2020: pre-restrictions between week 1, 2015 and week 12, 2020, and post restrictions-implementation between week 13, 2020 and week 52, 2021. We conducted an interrupted time-series analysis, assessing differences in <i>C. parvum</i> and <i>C. hominis</i> incidence, trends and periodicity between these periods using negative binomial regression with linear-splines and interactions. <b>Results</b> There were 21,304 cases between 01 January 2015 and 31 December 2021 (<i>C. parvum</i> = 12,246; <i>C. hominis</i> = 9,058). Post restrictions-implementation incidence of <i>C. hominis</i> dropped by 97.5% (95%CI: 95.4%-98.6%; p&lt;0.001). The decreasing incidence-trend observed pre-restrictions (IRR=0.9976; 95%CI: 0.9969-0.9982; p&lt;0.001) was not observed post restrictions-implementation (IRR=1.0081; 95%CI: 0.9978-1.0186; p=0.128) due to lack of cases. No periodicity change was observed post restrictions-implementation. Where recorded, 22% of <i>C. hominis</i> cases had travelled abroad. There was also a strong social gradient, with those who lived in deprived areas experiencing a higher proportion of cases. This gradient did not exist post restrictions-implementation, but the effect was exacerbated for the most deprived: 27.2% of cases from the most deprived decile compared to 12.7% in the pre-restrictions period. For <i>C. parvum</i>, post restrictions-implementation incidence fell by 49.0% (95%CI: 38.4%-58.3%; p&lt;0.001). There was no pre-restrictions incidence-trend (IRR=1.0003; 95%CI: 0.9997-1.0009; p=0.322) but a slight increasing incidence-trend existed post restrictions-implementation (IRR=1.0071; 95%CI: 1.0038-1.0104; p&lt;0.001). A periodicity change was observed for <i>C. parvum</i> post restrictions-implementation, peaking one week earlier in spring and two weeks later in autumn. Where recorded, 8% of <i>C. parvum</i> cases had travelled abroad. The social gradient observed for <i>C. parvum</i> was inverse to that for <i>C. hominis</i>, and was stable pre-restrictions and post restrictions-implementation. <b>Conclusion</b><i>C. hominis</i> cases were almost entirely arrested post restrictions-implementation, reinforcing that foreign travel is a major driver of seeding infections. <li><strong>Hospital strain and Covid-19 fatality: analysis of English nationwide surveillance data</strong> -

Objectives: To examine whether and to what extent hospital strain will increase the risk of death from Covid-19. Design: Retrospective cohort study. Setting: England. Participants: Data on all the 147,276 Covid-19 deaths and 601,084 hospitalized Covid-19 patients in England during the period between 9 April 2020 and 11 March 2022 were extracted on a daily basis from the UK Health Security Agency. Main outcome measures: The number of Covid-19 patients currently in hospitals was used as the measure of hospital strain. Daily case fatality was estimated as the measure of risk of death from Covid-19. The study was divided into 4 periods, which represented largely the wild, Alpha, Delta and Omicron waves. Weighted linear regression models were used to assess the association between hospital strain and Covid-19 fatality with adjustment for potential confounders including vaccination score, hospital admission rate, percentage of deaths outside hospitals, study period and interaction between patients currently in hospitals and study period. Results: The daily case fatality from Covid-19 increased linearly as the number of patients currently in hospitals increased in the 4 study periods except the Omicron wave. After adjusting for potential confounders, an increase in 1000 patients currently in hospitals was associated with a relative increase of 6.3% (95% CI: 5.9%~6.8%), 1.4% (95% CI: 1.3% ~ 1.5%) and 12.7% (95% CI: 10.8%~14.7%) in daily case fatality during study periods 1, 2 and 3 respectively. Compared with the lowest number of patients currently in hospitals, the highest number was associated with a relative increase of 188.0% (95% CI: 165.9%~211.6%), 69.9% (95% CI: 59.0%~81.8%) and 58.2% (95% CI: 35.4%~89.0%) in daily case fatality in the first 3 study periods respectively. Sensitivity analyses using the number of patients in ventilation beds as the measure of hospital strain showed similar results. Conclusions: The risk of death from Covid-19 was linearly associated with the number of patients currently in hospitals, suggesting any (additional) effort to ease hospital strain or maintain care quality be beneficial during large outbreaks of Covid-19 and likely of other similar infectious diseases.

<li><strong>Evaluation of S/F94 as a proxy for COVID-19 severity</strong> -

<p data-aos="fade-left" data-aos-anchor-placement="bottom-bottom">
Optimising statistical power in early-stage trials and observational studies accelerates discovery and improves the reliability of results. Ideally, intermediate outcomes should be continuously distributed and lie on the causal pathway between an intervention and a definitive outcome such as mortality. In order to optimise power for an intermediate outcome in the RECOVERY trial, we devised and evaluated a modification to a simple, pragmatic measure of oxygenation function - the SaO2/FIO2 (S/F) ratio. We demonstrate that, because of the ceiling effect in oxyhaemoglobin saturation, S/F ceases to reflect pulmonary oxygenation function at high values of SaO2. Using synthetic and real data, we found that the correlation of S/F with a gold standard (PaO2/FIO2, P/F ratio) improved substantially when measurements with SaO2 &gt; 0.94 are excluded(Spearman r, synthetic data: S/F: 0.31; S/F94: 0.85). We refer to this measure as S/F94. In order to test the underlying assumptions and validity of S/F94 as a predictor of a definitive outcome (mortality), we collected an observational dataset including over 39,000 hospitalised patients with COVID-19 in the ISARIC4C study. We first demonstrated that S/F94 is predictive of mortality in COVID-19. We then compared the sample sizes required for trials using different outcome measures (S/F94, the WHO ordinal scale, sustained improvement at day 28 and mortality at day 28) ensuring comparable effect sizes. The smallest sample size was needed when S/F94 on day 5 was used as an outcome measure. To facilitate future study design, we provide an online user interface to quantify realworld power for a range of outcomes and inclusion criteria, using a synthetic dataset retaining the population-level clinical associations in real data accrued in ISARIC4C https://isaric4c.net/endpoints. We demonstrated that S/F94 is superior to S/F as a measure of pulmonary oxygenation function and is an effective intermediate outcome measure in COVID-19. It is a simple and non-invasive measurement, representative of disease severity and provides greater statistical power to detect treatment differences than other intermediate endpoints.

Objective: To estimate vaccine effectiveness (VE) for preventing COVID-19 hospital admission in women first infected with SARS-CoV-2 during pregnancy, and assess how this compares to VE among women of reproductive age who were not pregnant when first infected. Design: Population-based cohort study using national, linked Census and administrative data. Setting: England, United Kingdom, from 8th December 2020 to 31st August 2021. Participants: 815,4777 women aged 18 to 45 years (mean age, 30.4 years) who had documented evidence of a first SARS-CoV-2 infection in NHS Test and Trace data or Hospital Episode Statistics. Main outcome measures: A hospital inpatient episode where COVID-19 was recorded as the primary diagnosis. Cox proportional hazards models, adjusted for calendar time of infection and sociodemographic factors related to vaccine uptake and risk of severe COVID-19, were used to estimate VE as the complement of the hazard ratio for COVID-19 hospital admission. Results: Compared with unvaccinated pregnant women, the adjusted rate of COVID-19 hospital admission was 76% (95% confidence interval 69% to 82%) lower for single-vaccinated pregnant women and 83% (75% to 88%) lower for double-vaccinated pregnant women. These estimates were similar to those found for non-pregnant women: 79% (76% to 81%) for single-vaccinated and 82% (80% to 83%) for double-vaccinated. Among those vaccinated more than 90 days before infection, being double-vaccinated was associated with a greater reduction in risk than being single-vaccinated. Conclusions: COVID-19 vaccination is associated with reduced rates of severe illness in pregnant women infected with SARS-CoV-2, and the reduction in risk is similar to that for non-pregnant women. Waning of vaccine effectiveness occurs more quickly after one dose of a vaccine than two doses.

Background: Since late 2021, the highly transmissible SARS-CoV-2 Omicron variant has driven a new surge of infections across the world. We used a case-ascertained study to determine the features of household transmission of SARS-CoV-2 Omicron variant in Shanghai, China. Methods: We collected detailed information on 323 pediatric cases and their 951 household members in April 2022 during the Omicron outbreak. All household members received consecutively intensive RT-PCR testing for SARS-CoV-2 and routine symptom monitoring within 14 days after exposure to a confirmed case. We described the characteristics of study participants and estimated the transmission parameters. Both secondary infection attack rates (SARI) and secondary clinical attack rates (SARC) among adult household contacts were computed, through which the transmission heterogeneities in infectivity and susceptibility were characterized and the vaccine effectiveness were estimated. Results: We estimated the mean incubation period of SARS-CoV-2 Omicron variant to be 4.6 (median: 4.4, IQR: 3.1-6.0) days and the mean serial interval to be 3.9 (median:4.0, IQR: 1.4-6.5) days. The overall SARI and SARC among adult household contacts were 77.11% (95% confidence interval [CI]: 73.58%-80.63%) and 67.03% (63.09%-70.98%). We found higher household susceptibility in females, while infectivity was not significantly different in primary cases by age, sex, vaccination status and clinical severity. The estimated VEs of full vaccination was 14.8% (95% CI: 5.8%-22.9%) against Omicron infection and 21.5% (95% CI: 10.4%-31.2%) against symptomatic disease. The booster vaccination was 18.9% (95% CI: 9.0%-27.7%) and 24.3% (95% CI: 12.3%-34.7%) effective against infection and symptomatic disease, respectively. Conclusions: We found high household transmission during the Omicron wave in Shanghai due to asymptomatic and pre-symptomatic transmission in the context of city-wide lockdown, indicating the importance of early detection and timely isolation of SARS-CoV-2 infections and quarantine of close contacts. Marginal effectiveness of inactivated vaccines against Omicron infection poses great challenge for prevention and control of the SARS-CoV-2 Omicron variant.
